CGDP Online Registration Form (Removed 9-29-17)


Important message concerning the CGDP Online Registration Form

As the Coverage Gap Discount Program (CGDP) system of record, sponsors must use CMS’ Health Plan Management System (HPMS) at www.hpms.cms.gov to provide up to date information on contract and contact information. Therefore, the TPA has removed the CGDP Online Registration form.
 
If your company’s contact information requires an update, the changes must be entered in the HPMS to reflect the correct personnel, such as the current TPA Primary Contact, or TPA Liaison - as the role is listed in HPMS. Once the TPA Liaison (primary contact) is updated, you must notify the TPA for further instructions.
 
Part D Sponsors should make sure that the data displayed in HPMS is the most current information and reflects the correct personnel listed for the following fields:

  • HPMS field “TPA Liaison”
  • HPMS field “Coverage Gap Discount Program (CGDP) Payment Contact”
